Culture Change
The Illinois Foundation for Quality Health Care’s Nursing Home Person-Directed Care (PDC) Change Package helps you to facilitate and track your progress toward improving quality of care and quality of life in your facility by following a person-directed care model. It incorporates the work of the Pioneer Network, Eden Alternative, Wellspring Model, and PEAK Initiative.
The Change Package presents six domains of focus for your efforts: resident control, leadership, care practices, workplace practices, environment and community. Each domain includes several key change ideas you can practice to promote growth in your nursing home culture.
